CLIENT AND DOG INFORMATION

TRAINING AGREEMENT

Welcome! We are so glad that you’re here and are entering a training program offered by AlphA and Omega Dog Training, LLC.

Our training programs focus on training you how to effectively communicate with your dog. Your trainer is going to teach you Leash Application and Handling Management Techniques, which will ensure a happier relationship with your dog for their entire life. For these lessons, you agree that you are going to be the Primary Handler of your dog. You agree to practice and implement the communication techniques consistently throughout the course of the training program.

You understand that you are not assigned any specific trainer and that we reserve the right to provide a different trainer during the course of your program, based on our operational needs.

Session Assessment:

You will be evaluated during each training session for proficiency in leash handling and consistency of handling management. At the end of your third session, the trainer will make an assessment as to the progression of your Leash Application/Handling Management. You must have a basic grasp and working knowledge of how to effectively communicate with your dog to proceed with any distraction training.

Guarantee:

AlphA and Omega Dog Training, LLC, guarantees that we will accomplish the goals we agreed to for training and will teach you how to get the same results in our follow-up handling sessions. Remember, you are required to follow the prescribed training regimen, or ‘homework.’ In the event you deviate from the training system, techniques, implementation, or consistency of commands, AlphA and Omega Dog Training, LLC, will not, does not, and cannot guarantee the results. We have 100’s of success stories and feel confident if you do your homework, you will be thrilled with the outcome for your Pack.

No Refunds, No Refunds, No Refunds.

No refunds will be given once the first session has commenced. AlphA and Omega Dog Training, LLC, is committed to the successful training of you (Client) and your companion. Our professional trainers will make every reasonable effort to teach you and your dog to master the skills listed in the course curriculum. However, we do not make any guarantee of the dog's future performance or behavior as a result of providing training, as it is the Client's duty to practice and reinforce the commands taught. Client understands that without practicing and reinforcing behaviors, their dog will naturally become less reliable over time.

TERMS AND CONDITIONS:

Owner’s Representations Regarding the Health of the Dog:

My dog is healthy! They are current with all required vaccinations, do not have any medical conditions that I am aware of except for those communicated, and is licensed with the appropriate licensing authority.

Tools, Techniques, and Training Plan:

The tools and techniques utilized by the AlphA and Omega Dog Training, LLC, team have been explained to me, and I understand and agree to the use of these tools and techniques in a manner consistent with the company’s training philosophy. AlphA and Omega Dog Training, LLC, believes in the humane treatment of all animals regardless of species. It is in this spirit that I agree that I will work to train my dog with compassion and patience.

AlphA and Omega Dog Training, LLC, will provide me with a training plan, household management guidelines, and practice exercises to do with my dog between sessions. I understand that the ultimate success of the training sessions is highly dependent on the degree to which I follow through with the recommendations and exercises prescribed to me by AlphA and Omega Dog Training, LLC.

Behavioral/Training Issues Disclosure and Goals:

I do hereby enter an agreement with AlphA and Omega Dog Training, LLC, to have my dog(s) trained in an Obedience Training program. The program in which I have chosen to have my dog trained requires active participation on my behalf. I understand that training my dog requires me to be consistent with training techniques and handling management outside of the scheduled training sessions.

Furthermore, I hereby acknowledge that I have completed the evaluation process, been given the opportunity to address my goals, and been counseled on the training methods, techniques, and philosophies utilized by AlphA and Omega Dog Training, LLC, and agree to maintain a standard of training that is consistent with the techniques shown to me throughout my training sessions. This includes the proper application of training collars, e-collars, leather leashes, and crates/kennels.

INDEMNITY RELEASE:

AlphA and Omega Dog Training, LLC, will endeavor to create as safe an environment as possible for the training and boarding care of my dog and will offer only sound, safe and responsible training, care, and post-boarding training instructions. I recognize that AlphA and Omega Dog Training, LLC, is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any product or service is not a guarantee of my satisfaction with that product or service. I understand the inherent risks in owning a dog, including but not limited to the risk of dog bites to myself or other people or animals. AlphA and Omega Dog Training, LLC, agrees to indemnify and hold me harmless from and against any claims of injury, expense, costs, loss, or damages caused by the gross negligence or willful misconduct of AlphA and Omega Dog Training, LLC, while my dog is in the sole custody, care, and control of AlphA and Omega Dog Training, LLC. I acknowledge that AlphA and Omega Dog Training, LLC, has no control over my dog when it is not in the exclusive custody, care, and control of AlphA and Omega Dog Training, LLC, and I agree to indemnify and hold AlphA and Omega Dog Training, LLC, harmless from and against any and all claims of injury, expense, costs, loss, or damages caused by the actions of my dog when it is not in the sole custody, care, and control of AlphA and Omega Dog Training, LLC.

PHOTO/VIDEO RELEASE:

I grant AlphA and Omega Dog Training, LLC, the right and permission to use, reproduce, distribute, and publish any photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s), and/or sound recording(s) made of me and/or my dog at any time during the training and/or boarding of my dog, and I hereby release AlphA and Omega Dog Training, LLC, from any and all liability resulting from such use, reproduction, distribution, and publication. I authorize the reproduction, sale, distribution, copyright, exhibition, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of AlphA and Omega Dog Training, LLC, and I specifically waive any right to compensation I may have for any of the foregoing.

CREDIT CARD AUTHORIZATION:

AlphA and Omega will not record, store, or save any card information in our records. We do not auto-bill clients. It is the clients' responsibility to make payments when due according to the payment schedule. It is my wish to pay my training fees and costs by using my major credit card, and AlphA and Omega Dog Training, LLC, has consented to accept payment of such funds via credit card as an accommodation to me as a client. I understand that in using my credit card to pay my training fees and costs that I am bound by the terms of this agreement with regard to fee disputes, and I knowingly waive any right of dispute that I may have under state or federal law to dispute this charge made by AlphA and Omega Dog Training, LLC, against my card. In the event that any charge is disputed, a copy of this agreement will be presented to the credit card processing company and will serve as conclusive proof that the charge was authorized and may be relied upon by the credit card processing company to dismiss the dispute as unfounded. If any charge made against my card is dishonored, declined or reversed by the credit card company, for whatever reason(s), I agree to make other suitable payment arrangements with AlphA and Omega Dog Training, LLC, within 72 hours of receiving notice of such. I understand that failure to do so may result in AlphA and Omega Dog Training, LLC’s, termination of the training program, and collections actions being initiated against me.

SIGNATURES:

By signing manually or electronically below, I acknowledge this contract contains the entire agreement between the parties. I acknowledge and represent that I have carefully read the Training Agreement and the Terms and Conditions, understand it, and sign it voluntarily. In the absence of my signature on this document, any payment toward goods or services will imply I am in full agreement with this contract. No oral representations, statements, or inducements, apart from the foregoing written agreement, have been made. I certify the accuracy and completeness of all information provided by me, and I agree to the terms and conditions herein. I am the legal owner of the dog(s) which is the subject of this agreement; I am of legal age and fully competent and I execute this agreement for full, adequate, and complete consideration fully intending to be bound by the same.
